Phase 2 is now open! Please download the form from here, fill it up, save it, and email it to pidc2012@gmail.com. Kindly append your institution acronym to the end of the file name (i.e. Phase 2 Form UPD.pdf) You need Adobe Reader to view this file. Please email us if you experience any difficulties while filling out the form.

Also, please make sure that you send one ID photo per participant. The requested format is indicated in the infographic below.
